I currently live in Canada, Canadian citizen. However, I was born in Italy. I've decided that when I'm 21 I'm gonna change my first and last name legally, I will change my last name to my mom's. But will I still need my parent's permission to change my birth certificate name even though I have already changed my name legally.

It's my understanding that you cannot change what's on your birth certificate, no matter where you were born. You'll still have your old name there, and new paperwork showing the legal change from that name to your new name. It's a similar situation as getting married - you'll still have paperwork in the old name proving your birth.
